Forms and Applications


To open some of the documents you will need Adobe Acrobat Reader.  Adobe is available to you as a free download from http://www.adobe.com/products/acrobat/readstep.html.  The forms have an inserted text box that allows you to type directly in the form instead of printing the form off and hand writing or typing in the information.

Please follow these steps in completing the forms.

1 - Choose the form or application you wish to complete.  (Depending on the speed of your internet connection, this may take several minutes.  Please be patient.)

2 - When the form is open, you may choose to save it or fill it out and print immediately.  (If you fill it out, please be aware that Acrobat Reader does not allow you to save the form with the information in it, only the form itself.)

 3 - If you choose to save the form, go to File, Save as and save the form either to a disk or to your computer hard drive.  (Remember to note where you save the file so you can find it again.)

4 - After you save the form, you may disconnect from the internet.

5 - Remember where you saved the form?  Go ahead and open it.

6 - Complete the form.  (Remember that Acrobat Reader will not allow you to save the form with information typed in it.)
